A signed dog grooming waiver is the single most important piece of paperwork your salon will ever produce, and most groomers either don't use one or use one they cobbled together from Google. Both leave you exposed. A real waiver covers the specific risks of grooming (matted coats, senior pets, behavioral incidents, accidental nicks), establishes your policies in writing, and gives you a documented signature you can point to if anything ever goes sideways. This template is built specifically for grooming operations and includes the standard liability language, policy acknowledgments, and signature blocks you should have in place before you take your next first-time client.
Three reasons, in order of importance:
Legal protection. Without a signed waiver, you have minimal documentation that the client acknowledged the inherent risks of grooming. If a senior dog has a medical event during a groom or a reactive dog bites a bather, your defense gets dramatically harder.
Policy enforcement. A waiver is how you formally document your cancellation policy, photo permissions, payment terms, and matted coat shave-down authorization. Verbal agreements at the front desk don't hold up later.
Professional standard. Every reputable grooming business uses a waiver. Skipping it signals to clients that you're not running a real operation.
A complete waiver addresses six categories of risk and policy. Each section below is template language you can copy and adapt to your salon, mobile unit, or home studio.

By signing below, I acknowledge and agree to the terms outlined in this waiver for grooming services performed by [Your Salon Name].
I, the undersigned owner or authorized agent of the above-named pet, authorize [Your Salon Name] to perform grooming services, including but not limited to: bathing, drying, brushing, clipping, scissor work, nail trimming, ear cleaning, sanitary trimming, and other related services as agreed upon at the time of appointment.
I understand that grooming services involve handling, restraint, and the use of professional grooming tools. I authorize the use of standard grooming equipment and products on my pet.
I confirm that my pet is current on all required vaccinations, including rabies (as required by state and local law). I understand that [Your Salon Name] may request proof of vaccination at any time and may refuse service if current vaccinations cannot be verified.
I understand that bringing an unvaccinated pet onto the premises may pose a health risk to other pets in care and is my responsibility.
I have provided accurate and complete information regarding my pet's medical history, including:
I understand that failure to disclose accurate medical information may result in adverse outcomes for my pet, and I will not hold [Your Salon Name] responsible for medical complications arising from undisclosed conditions.
I have honestly disclosed all known behavioral information about my pet, including any history of:
I understand that grooming is stressful for some pets and that [Your Salon Name] reserves the right to discontinue service at any time if my pet poses a safety risk to staff or itself. In such cases, I may be charged for the time spent and any services completed.
I understand that failure to disclose behavioral history may result in injury to staff, other pets, or my own pet, and I will be financially responsible for any resulting damages or medical expenses.
I understand that severely matted coats can only be safely addressed by shaving down to or below the matting. I authorize [Your Salon Name] to shave my pet's coat as necessary for safety and humane treatment.
I acknowledge that demating or shaving severely matted coats may result in:
I will not hold [Your Salon Name] responsible for these outcomes when shave-down is performed for the safety and welfare of my pet. I understand additional fees may apply for matted coat services and will be quoted before the work begins when possible.
For pets aged 10+ years, brachycephalic breeds (Pugs, Bulldogs, Boston Terriers, etc.), pets with known cardiac or respiratory conditions, or pets with other underlying health concerns, I acknowledge that:
I will not hold [Your Salon Name] responsible for medical events that occur when accurate medical history has been disclosed and reasonable care has been provided.
I understand that despite professional care, the following can occasionally occur during grooming:
I understand these outcomes are rare, and that [Your Salon Name] takes reasonable precautions to prevent them. I will not pursue legal action against [Your Salon Name] for routine, minor incidents that are consistent with industry-standard care.
I understand and agree to the following cancellation policy:
I authorize [Your Salon Name] to charge the credit card on file for no-show or late cancellation fees as outlined in this policy.
☐ I grant [Your Salon Name] permission to photograph my pet for marketing purposes, including but not limited to social media, website content, and promotional materials.
☐ I do NOT grant photo permission.
I understand I can revoke or change this permission at any time by notifying [Your Salon Name] in writing.
I understand that payment is due at time of service. I agree to pay all charges for services rendered, including any agreed-upon surcharges for matted coats, difficult handling, specialty services, or add-ons.
I authorize [Your Salon Name] to charge the credit card I provide for services rendered, no-show fees, and any agreed-upon charges.
In the event of a medical emergency, I authorize [Your Salon Name] to seek emergency veterinary care for my pet at the nearest available animal hospital. I understand that I am financially responsible for any veterinary expenses incurred.
I will not hold [Your Salon Name] responsible for the cost of emergency veterinary care necessitated by undisclosed medical conditions or behavioral incidents.
I have read, understood, and agree to the terms of this waiver. I confirm that I am the legal owner of the pet listed above, or authorized to make decisions on behalf of the owner.

Three options:
Build the waiver into your grooming software as a digital signature form. Teddy, MoeGo, DaySmart, and Gingr all support digital intake forms and service agreements with e-signatures. Signed copies live on the pet profile for easy reference.
If you're creating a complete onboarding process, pair this waiver with a Dog Grooming Intake Form Template to collect pet history, behavioral information, and service preferences before the appointment.
Convert this template to a PDF, print copies, and have new clients sign at first visit. Scan and store the signed copies digitally.
Use Google Forms, Jotform, or DocuSign Free to build a digital version. Email the link to new clients before their first appointment.
You can decline service. Most professional grooming businesses require waiver completion for first-time clients. Politely explain that the waiver protects both the pet and the salon. If a client refuses, decline gently and recommend they look elsewhere.
A client who refuses to sign a standard liability waiver is signaling future problems. Better to not start the relationship.
Review your waiver annually. Update sooner if:
For meaningful changes, have all existing active clients re-sign the updated version.
Many salons also maintain a separate service agreement alongside their waiver. If you need one, see this Pet Grooming Service Agreement Template for a companion document covering business policies and service terms.
This template is provided as a starting point and does not constitute legal advice. Have your final waiver reviewed by a licensed attorney in your state to ensure it complies with local laws and adequately protects your business. Waiver enforceability varies by state, and certain language may need to be added or modified for your jurisdiction.
There's no federal law requiring a grooming waiver, but operating without one exposes your business to significant legal and financial risk. Every reputable grooming business uses one. Many states' liability case law effectively requires documented client acknowledgment of grooming risks.
An intake form gathers information about the pet (breed, behavior, medical history, service preferences). A waiver is a legal document where the client acknowledges risks and agrees to your policies. Many salons combine both into a single document at first visit.
For a complete onboarding workflow, many groomers use both a waiver and an intake form together.
Yes, a single waiver can cover all life stages if it includes specific senior pet and at-risk pet sections (as this template does). Some salons use age-specific addendums for puppies and seniors that prompt for additional disclosure.
Digital storage in your grooming software is the easiest and most secure option — signed copies live on the pet profile, automatically backed up and easy to retrieve. If you store paper copies, keep them in a locked filing cabinet and retain for at least 5 years (or per your local statute of limitations).
Mobile groomers should add a section to their waiver acknowledging risks specific to mobile service: greywater disposal, parking on private property, restrictions on entering the van, and the limitations of working in a confined space. Otherwise the core template applies.
